Important Information Before You Register:

You must carefully and completely enter all information into the required fields in the registration form before it will allow you to submit. If you do not complete a required field, you will have to begin the registration process again. Please take your time and take note of required fields. In addition, please make every effort to gather necessary information to complete this form (donor addresses, name of your Collectible, etc.) If you do not have the information necessary to complete a required field, please type "Don't Know" into the text box.

This information is for the sign placed by your Collectible. Accuracy is essential. Signs will be printed prior to Decorating Day; therefore, it is important you confirm participants and the spelling of their names. You may list up to three decorators and donors. Organizations or group names are encouraged instead of individuals.
