If you do not complete a required field, you will have to begin the registration process again. Please take your time and take note of required fields. In addition, please make every effort to gather necessary information to complete this form (donor addresses, name of your playhouse, etc.)

This information is for the sign placed by your Playhouse. Accuracy is essential! Signs will be printed prior to Decorating Day; therefore, it is important you confirm participants and the spelling of their names. You may list up to three decorators and donors. Organizations or group names are encouraged instead of individuals.
